Abbey Hospice is currently seeking to hire a full time LMSW. This is a Monday-Friday position, with occasional weekend on-call responsibilities.
The Social Worker is responsible for provision of supportive services to help meet patient/family social, emotional and economic needs relative to the impact of terminal illness. The Social Worker is administratively responsible to the Patient Care Coordinator.
Responsible for social service activity as it relates to terminally‑ill patients and their families. Responsible for serving as a liaison between Abbey Hospice and the social work community. Responsible for maintaining clinical expertise in social work care of the terminally‑ill patient.
Education. Preparation and/or experience: Educated with a Master’s Degree in Social Work from a school accredited by the National Association of Social Workers. Minimum of two years experience in health care or related field. Supervisory experience preferred. Hospice experience preferred. Demonstrated knowledge and skills in family dynamics, psychosocial assessment and needs, crisis intervention and individual and family counseling. Knowledge and skill in the appropriate use of community resources, including local, state, federal, and other agencies.
Interpersonal and Communication Skills: Proven ability to work with multi‑disciplinary team with minimal supervision. Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals. Ability to write routine reports and correspondences. Ability to speak effectively before groups of customers or employees of organizations. Ability to relate to people with diverse backgrounds, skills and abilities.
