The Alliance for Better Health Care is a collaborative network of more than 2,000 providers and community-based organizations who are partnering to transform how health care is delivered in the Capital / Tech Valley region, with a focus on serving an estimated 193,000 Medicaid members and uninsured individuals. We as part of the Delivery System Reform Incentive Payment Program (DSRIP) through the NYS Department of Health, and our goal is to achieve the so-called “Triple Aim” of improving patient care: improving the overall health and wellness of the communities we serve, and reducing health care costs by reducing the number of avoidable hospitalizations and emergency department visits by 25% over a five-year period.
The Innovative Health Alliance of New York, LLC. (IHANY) is a collaboration among doctors, health systems and health care providers in New York’s Capital Region aimed at improving the health of the communities they serve by working together in new, more coordinated ways. In today’s evolving health care landscape, clinical integration is essential to achieving the three-part aim of better health for the community, better care for individuals and lower costs of health care. IHANY is the Capital Region’s first Clinically Integrated Network (CIN). This physician-led endeavor is designed to control health care costs and ensure patient care quality by enabling physicians, health systems, such as Ellis Medicine, St. Peter’s Health Partners, St. Mary’s Healthcare, Amsterdam, and other health care providers, like Hometown Health, to work together in joint contracting and share incentive-based payments. – less
