Location: Areas to Cover - (2) positions available; one to cover West Side full time: (2nd) position to cover North Phoenix perdiem.
Responsible for providing home care services consistent with the mission, vision and goals of the Agency. The RN is responsible for providing skilled nursing services under a multi-disciplinary plan of care approved by the patient's physician. Nursing care includes, but is not limited to, patient assessment, evaluation and coordination of the medical / nursing needs of the patient; performance of prescribed medical treatments; and instruction of caregivers in the provision of care to the patient. The RN shall provide for the continuity of services for the patient and his/her family.
DUTIES AND RESPONSIBILITIES:
Complete initial and on-going comprehensive assessments of the patient’s needs including OASIS assessments at appropriate time points.
Identify the patient and family’s physical, psychosocial, environmental, safety and developmental needs and initiate an individualized patient plan of care based on the needs identified.
Revise the plan of care as necessary.
Provide service and /or treatments requiring substantial and specialized nursing skills.
Initiate appropriate preventative and rehabilitative nursing procedures.
Counsel, support and instruct the patient, family and caregiver in meeting nursing and related needs.
Refer to other services as needed.
Document nursing assessments, problems identified, appropriate goals, care plan, interventions provided and evaluation of patient / family outcomes achieved .
Coordinate all patient services.
Inform the physician and other appropriate staff of changes in the patient needs / condition.
Support the patient’s and family’s cultural beliefs.
Perform duties consistent with the Nursing Practice Act including the Standards of Competent Performance.
Demonstrate the ability to correctly use and direct the use of medical supplies and equipment.
Supervise care given by the para-professional, Home Health Aide, and Licensed Practical Nurse providing services to the patient as required by law and licensure. Supervision is documented on the Nursing Visit Note.
Assign Home Health Aide (HHA) to a specific patient and prepare Assignment sheet.
Plan for discharge from service.
Participate in the On-Call rotation as requested.
Participate in orientation, in-service education, utilization review and patient care conferences.
Prepare and submit, as directed, all case documentation, including clinical and progress notes, and required payroll information according to Agency policy.
EDUCATION, EXPERIENCE, AND TRAINING
Minimum of one (1) year experience in Home Healthcare and Oasis knowledge!
Current, active license in the state of Arizona Board of Nursing, pursuant to ARS Title 32, Chapter 15 including fingerprint checks.
Current CPR, health clearance, TB clearance.
Successful passage (80%) of Agency competency exam, clinical skills self-assessment and core competency per skills checklist.
