Aunt Ann's Home Care is seeking a CareManager- Home Care Supervisor for our San Francisco/ San Mateo County Service Area. Since 1978, Aunt Ann's Home Care has employed a RN and/or LVN to assess, develop an individual care plan specific to our clients ongoing needs, and supervise the caregivers who are employees of Aunt Ann's Home Care in our clients home. We are seeking a new member of our office team to fill this position.

Our clients are primarily seniors who want to age in place and seniors who are recovering from an illness and need temporary help. Some of our clients have received care for over 20 years from our caregivers.
This is a critically important position in our organization. The Care Manager/ Home Care Supervisor will meet with the client and/or clients family and representative in their residence, assess the care needed, make recommendations for staffing and upon hire of the agencies caregivers, the Home Care Supervisor- Care Manager will develop an individual care plan .
The Care Manager / Home Care Supervisor will also supervise the caregivers on a periodic and regular basis and document their observations.

We were the first agency to run background checks on ALL caregivers in 1988, and don't refer independent contractors. We invest in our caregivers and are seeking someone who is passionate about senior home care, and investing in the education of our caregivers to improve the quality of care delivered. We also would like to hire a person who can teach the agency orientation and on going skill training program. ( Optional requirement but desired)
Qualifications: One year professional home care supervisory or non-medical home care experience . LVN preferred, RN or MSW acceptable. Must have a reliable vehicle for home/hospital visits, supervision and assessments.
This can be a full time or part time benefitted position.
