Community Living Connections is an exciting non-profit organization that provides residential and community based supports to adults with developmental disabilities who live in and around the Madison area.
Skills Teachers provide support to consumers in the consumer's home/apartment and in the community. They assist and teach activities of daily living, coordinate and accompany people on recreational activities and provide companionship.
Part Time and Full Time positions available in Madison, Mt Horeb and Stoughton. A variety of shifts are available including mornings, evenings and weekends.

A vehicle, valid WI driver’s license, proof of adequate auto insurance and acceptable driving record are required. CLC requires proof that auto insurance meets one of the following requirement: A split limit of $250,000/$500,000 for bodily injury OR A combined single limit of $300,000 for bodily injury and property damage OR A separet personal umbrella policy presented with proof of auto insurance.
Prior experience working with people with disabilities is beneficial but not required. Paid training is provided.

COMMUNITY LIVING CONNECTIONS, INC.
Under the supervision of Community Support Managers and lead staff of the specific program site, Skills Teachers provide necessary supports in daily living to persons with developmental disabilities, including care, treatment, habilitation and community integration. Major responsibilities include but are not limited to the implementation and monitoring of specific goals as reflected in each consumer's individualized case plan. Although assigned to a specific residential site, on occasion Skills Teachers may be reassigned on an as needed basis to a different site should program needs require it.
Examples of Work Performed
-Implement prescribed plans of care, including goals and objectives reflected in the individualized case plan and as assigned by supervisory staff.
-Foster the individual growth and development of each consumer, including fostering the greatest possible community integration and the ability to independently perform activities of daily living.
-Assist in the development of daily and weekly routines and schedules for consumers and staff.
-Promote and assist consumers in utilizing community resources.
-Monitor and assist in maintaining a clean and safe home environment. This includes but is not limited to complying with Infection Control/Universal Precautions guidelines, maintaining sanitation standards, responding to health and safety crises.
-Provide guidance in and monitor consumers' personal appearance and hygiene. Provide as needed assistance to consumers in personal care.
-Teach consumers about and monitor menu planning, grocery shopping, food storage and food preparation so as to ensure a healthy and appropriate diet.
-Work hours are expected to be flexible as determined by program needs and emergency situations. These work hours include overnight shifts as necessary.
- Assist consumers in maintaining budgetary guidelines as assigned by supervisory staff.
-Keep records and make reports as required, both verbally and in writing. Inform lead program staff and Community Support Manager(s) on issues pertaining to consumers, including but not limited to any change in health or behavioral status.
-Effectively and appropriately communicate with families and consumers and all significant others as needed.
-Attend consumer appointments and/or meetings as assigned by supervisory staff.
-Assist Community Support Manager(s) in the development and review of individualized case plans.
-Attend staff and team meetings as scheduled by supervisory staff.
-Attend all required initial orientation and ongoing inservice training sessions.
-Adhere to assigned work schedule. Follow mandated procedure for requesting and/or informing supervisory staff about all scheduled and emergency deviations from assigned work schedule.
-Comply with all policies and procedures of Community Living Connections and Community Living Connections Programs.
-Consult with lead program staff, Community Support Manager(s), Outreach Coordinators, Residential Services Coordinator, and Division Director(s) on areas outside of authority and/or competence.
-Responsible for assisting the consumer in the up-keep of their apartment/house as needed. Skills Teacher will also adhere to all stipulations included in the consumer’s lease.

Training and Experience
