Coordinates preventive health care for Medicaid recipients from birth to age 20.
Serves as a liaison between the Medicaid recipient or responsible family member and physicians, other providers, professional organizations and agencies supporting preventive services in the community.
Makes follow-up contacts to families of Health Check children regarding guidelines for use of the Emergency Room and the medical home concept.
Identifies and educates the community resources for the client population and makes referrals to agencies as appropriate.
Increases community awareness of the Health Check/NC Health Choice Programs on a regular basis by organizing mailings, participating in health fairs, or other appropriate means of communication.
Identifies and supports Health Check providers, acting as a liaison between Health Check providers and the State Medicaid office, EDS and DMA.
Education, Training and Experience:
High school graduate required
Recent healthcare or public service experience preferred
Excellent interpersonal, communication and negotiation skills
Strong organizational and time management skills
Ability to work independently
Transportation required to meet the core job requirements
