As Clinical Care Coordinator, you will be responsible for the overall direction of home health and hospice clinical services. Duties include supervising nursing staff, accepting home health and hospice referrals, assuring proper maintenance of clinical records including electronic charting system, leading interdisciplinary team conferences, collaborating with area health agencies and providers, participating in patient/client care, and assuring compliance regarding program and staff licensure and certification.

Qualifications: Bachelor’s degree in nursing preferred. Experience in home health and hospice required. Pre-employment background check required.
