Position Title: Director of Patient Care Services

Primary Function:

The Director of Patient Care Services acts on behalf of the Administrator in her absence. She is the leader of the Patient Care Team and the member of the team whose function is to supervise, evaluate and coordinate the various component members of the interdisciplinary team, assure continuity of care from admission to discharge or transfer to bereavement, serves as patient advocate and coordinator for other social service and health care providers in the community who are involved in the care of the team’s patients, assumes responsibility for the maintenance of patient records from intake through discharge or transfer to bereavement.

Essential Job Responsibilities:

1. SUPERVISIONAL RESPONSIBILITIES

When assigned by the administrator, may hire, orient, supervise, train and evaluate the performance of the team/potential team members.

a. Directs all patient care services for the team, including volunteer and bereavement, and ensures that services are rendered as integrated components of the Interdisciplinary plan of care.
b. Assures efficient paper flow according to the operational systems so attendance, payroll, and billing documentation are available as needed.
c. Assures compliance with the policies and procedures of the hospice as well as regulatory and statutory requirement in the patient care area.
d. Authorizes all expenditures for patient care under the supervision of the Administrator.
e. Assures that the professional standards of community nursing practices are maintained by all nurses providing care.
f. Participates in enacting policies and procedures.
g. Assists in maintaining vendor contracts.
h. Implements Quality Improvement policies and oversees routine evaluations of charts.
i. Reviews information on all pending patient admissions, all new admissions and “alive” discharges to assure criteria are met.
j. Works in direct relation to Managed Care/Admission Coordinator.
k. Manages team schedule.
l. Participates in on-call rotation.
m. Reviews payroll and mileage and additional expenses prior to Administrator.
n. Acts as resource and mentor for staff regarding clinical issues, documentation, problem solving and appropriate customer service behavior.

2. QUALITY IMPROVEMEN/REGULATORY COMPLIANCE

a. Monitors clinical records and all team activities to ensure that they are accurate, complete and meet required standards in accordance with policy.
b. Performs utilization review of continuous care and inpatient levels of care for all patients on the team.
c. Assists with surveys and requests from regulatory bodies and intermediaries.
d. Performs chart reviews routinely to assure there is evidence that quality care is being delivered.

3. CUSTOMER SERVICE/SALES/MARKETING

a. Assures that problems/grievances/service failures experienced by individual patients/families or physicians IPAs are addressed with team members, vendors are resolved promptly and satisfactorily.
b. Regularly visits LTC facilities to assure care plan integration, customer satisfaction and to discuss criteria for determining appropriateness.
c. Provides in-services to customers along with other team members.

4. COST CONTAINMENT

d. Verfies/approves admitting IPOC diagnosis, treatments, staffing and supplies.
e. Monitors utilization of resources by every patient to assure cost effective delivery of services.
f. Approves all bills and invoices related to patient care services under the supervision of the Administrator.

5. PROFESSIONAL DEVELOPEMENT

a. Attends in-services, education seminars and workshops.
b. Develops and achieves professional growth goals and objectives.
c. Participates as a mentor of newly hired employees.

Qualifications

  • Reliable transportation with appropriate license and insurance coverage for driver and passengers.
  • Private telephone in home.
  • 5 years hospice experience
  • Registered Nurse in the State of California
  • Bachelors Degree
  • Successful supervisory experience
