What We Do
Communities of Care makes a measurable difference in the lives of families each day. We serve medically fragile children, teens, and adults. Our nurses provide extended-hours skilled care in clients' private homes in and around the 7-county Twin Cities metro area.
With our unique staffing and scheduling method, each nurse usually works with just one case. This allows nurse and client to establish a high level of trust--and that leads to better care. Every nurse has the opportunity to meet the client as part of the placement process. When both client and nurse agree to the match, everyone feels more comfortable.
Home care offers nurses the opportunity to work one-on-one with their clients. Our nurses report that this allows them to really get to know their clients and care for them at a comfortable pace.
