The Personal Care Coordinator will work directly with members and the internal and external health care team to coordinate medical and non-medical supportive care for our members. The Personal Care Coordinator will conduct necessary outreach (via home visits and telephone) to high risk members for education on compliance with written treatment plans; maintain accurate records of assigned cases, including hospital, outpatient, specialty, and ancillary services pertinent for member coordination; and notify all responsible parties of health care services received to ensure care coordination and management.
Candidate must have a Bachelor’s degree in health related field (i.e., social work, sociology, psychology, and health education); ability to communicate effectively verbally and in writing; exceptional telephone and customer service skills; be comfortable with visiting members in their homes and/or facilities; knowledge of managed care principles, CPT, ICD-9, HCPCS coding, Medi-Cal benefits and experience with inpatient and outpatient medical review guidelines (Milliman USA, Interqual); be bilingual (English/Spanish preferred; or English/Arabic or English/Vietnamese); and, have reliable transportation with current California driver’s license and proof of auto insurance.
