We currently have an excellent career opportunity for a Licensed Geriatric Specialist in one of our Wellness and Recovery Centers located in El Cajon. This is a regular full time position. The Heartland Center offers a wide range of outpatient mental health services to the culturally diverse adult population of San Diego's East Region. Heartland provides initial psychosocial assessments, psychiatric and medication evaluations, medication support services, limited individual supportive counseling, group counseling, care coordination, information, referrals and linkage, crisis intervention, and psychosocial rehabilitation activities. Heartland provides these services under contract with the County of San Diego Mental Health Services and the Health and Human Services Agency to individuals 18 years of age and older.
We have a specialty program for young adults (age 18-24) who present with unique needs and preferences in mental health services. We also have a specialty program for older adults (age 60 year and over) who present with a variety of mental health and medical needs.
The Geriatric Specialist provides home visits and off-site screenings, assessments, short-term counseling, crisis intervention, case management, and outreach activities within a psychosocial rehabilitation treatment model to Older Adult consumers (ages 60 years and over). S/he assists clients in accessing and using medical care, community resources, family and other support systems to achieve an optimal level of health and functioning in the community. S/he also provides linkage, outreach, and consultation to other service providers for the older adult population.
Qualifications/ Skills Requirements
The candidate is a Licensed Clinician (LCSW, Psy.D., Ph.D. or LMFT or CA Licensed RN). Preferred applicants: those with experience working with Older Adults who have Serious and Persistent Mental Illness, experience providing in-home services, knowledge of SD county paperwork / Anasazi, and with a walk-in model based program.
Basic computer skills are required as well as a valid State of California driver’s license in good standing and auto insurance coverage within 10 days of employment is required and must be maintained during course of employment.
