ENTRY-LEVEL - Qualified Mental Health Professional (QMHP)

EDUCATION:

  • (a) an individual who holds a license, provisional license, certificate, registration or permit issued by the governing board regulating a human service profession, a registered nurse who is licensed to practice in the State of Virginia by the Virginia Board of Nursing who also has four years of full-time accumulated experience in DBHDS with the population served; or
  • (b) a graduate of a college or university with a Master’s Degree in human service field and has one year full-time, post-graduate degree accumulated DBHDS experience with the population served or a substance abuse professional who has one year full time, post-graduate degree accumulated supervised experience in alcoholism and drug abuse counseling; or
  • (c) a graduate of a college or university with a Bachelor’s Degree in human service field and has two years full-time, post bachelor’s degree accumulated DBHDS experience with the population served, or a substance abuse professional who has two years of full-time, post-bachelor’s degree accumulated supervised experience in alcoholism and drug abuse counseling; or
  • (d) a graduate of a college or university with a Bachelor’s Degree in a field other than human services and has four years full-time, post bachelor’s degree accumulated DBHDS experience with the population served, or a substance abuse professional who has four years of full-time, post bachelor’s degree accumulated supervised experience in alcoholism and drug abuse counseling.
  • (e) a Certified Clinical Supervisor (CCS) and Certified Clinical Addiction Specialist (CCAS) may deliver CS as Qualified Professional.

DUTIES: To perform this job successfully, the individual must be able to perform each essentially duty satisfactorily. RESPONSIBILITIES:

  • Provides intensive in-home services to child & adolescents and their family, overseeing the Individual Service planning process for consumers, including designing interventions and strategies for the other team members.
  • Will be responsible for supervising other intensive in-home team members; oversee and manage authorizations, assist with determining medical necessity, assist teams in discharge planning and participate in a multi-regional collaborative as needed to troubleshoot implementation challenges; implement and maintain processes for efficient and sound reporting, filing, and management of service delivery and supervision.
  • Maintains contact with the LME’s, parents, guardians etc., and representatives of other community organizations to exchange and update information on need for services and availability of resources to provide Program Director is able to plan, organize and coordinate with LMHP all services consumers receive through Crestar Health LLC.
  • Coordinates and oversight of initial and ongoing assessment activities, the development, monitoring, revising, and updating of the consumer’s Individual Service Plan.
  • Consults with LME’s identified professionals, family members and others, include their input into the ISP process, inform all involved stakeholders, and monitor the status of the recipient and relationship to the treatment goals.
  • The coordination of movement across levels of care, directly to the recipient and their family; the coordination of discharge planning and community re-entry following hospitalization, residential services and other levels of care.
  • Provide direct face-to-face monthly supervision to all designated Associate Qualified Professionals as well as Paraprofessional according to supervision requirements and according to licensure and certificate requirements of the appropriate discipline.
  • To ensure Intensive in-Home staff is providing quality services to consumers.
  • To increase, promote, and maintain self-help, family and natural community supports
  • To provide the appropriate documentation for service delivery specified by Medicaid and Crestar Health LLC standards.
  • Additional duties will be assigned as program needs change.
  • QHMP’s are supervised and reports directly to the LMHP.

REQUIREMENTS:

  • Provide documentation daily
  • Valid Virginia Driver’s License
  • Personal vehicle insurance coverage
  • 20 hours of training specific to the required service definition components (including Person Centered Planning and Crisis Response) within the first 90 days of employment.

QUALIFICATIONS:

1. Exhibit core skills including: technical knowledge, cultural awareness, and analytical skills and clinical skills

4. No substantiated findings of abuse or neglect listed on the Virginia Healthcare Personnel Registry

5. Disclosure of any criminal conviction

6. No history of alcohol or drug abuse in the two years previou
