The company does not learn from the past so problems from the past keep resurfacing.
Managers are expected to quietly follow their managers, so real problems essentially get kicked under the rug. This happens until a bunch of people trip over a very large bump in the carpet and fall down. The real problem is the leadership never really questions why dirt was kicked under the rug in the first place.
And then they go back to doing what they were doing--which is quietly following their managers.
