We are Family Home Health Care, a licensed home health care agency. We provide excellent and compassionate services to clients who are in need of assistance with daily living activities in the comfort of their homes.

Currently, we are seeking a Director of Clinical Services professional to be based at ourTarrytown, NY location.

The Director of Clinical Services is an integral member of our agency and is responsible to organize and direct professional and paraprofessional services, is accountable for adherence to federal, state, and local laws, accreditation, and agency policies and standards for compliance.

POSITION SUMMARY:
==================
Carry out the agency’s mission, philosophy, goals and objectives within guidelines of agency policy and position function.

Interpret and implement the Agency’s philosophy to staff and members of the community.

Comply with accredited ethical conduct and professional standards of nursing practice set forth by the American Nursing Association.

Assist in planning overall development and administration of the agency under the direction of the Administrator

Participate in the development of administrative policies and procedures of the agency.

Direct implementation of approved work methods and procedures that reflect elements essential to rendering high quality care.

Participate in the review, analysis, and appraisal of the effectiveness of the total agency problem.

Ensure the accuracy of public information materials and activities.

Implementation of the agency’s professional and paraprofessional services and quality assurance programs.

Supervise the Nursing Supervisor and all professional personnel.

Select and maintain a qualified, well organized staff to provide care for the needs of the clients.

Establish methods for coordination of care by all disciplines.

Give leadership in promoting and maintaining standards for providing good quality by all members of the client team.

Supervise and teach to improve practice within the agency.

Consult with the appropriate Quality Improvement Committee member in matters pertaining to client care services.

Promote staff development by utilizing capabilities of nursing and paraprofessional personnel in the development of orientation and staff education programs.

Serve as a member of the Quality Improvement Committee.

Participate in local organizations and activities related to the health professions and community health services.

Participates in direct client care activities.

Document all client related activities in accord with procedural recording requirements and in a timely manner.

Respond to change productively and performs other job-related tasks and duties as assigned.

Participate in Performance Improvement activities as required.

Participate in evaluating overall position performance, goal setting and achievement, and performance improvement plan.

QUALIFICATIONS:
===============
Registered Nurse with a Baccalaureate or higher degree in nursing or other health related field and one year of supervisory or administrative experience within the last three years in a home health agency, or health facility.

OR

A Registered Nurse with two years or supervisory or administrative experience within the last four years in a home health or health facility and an additional two years of clinical experience.

Must be currently licensed (in good standing) in New York State.

Must meet educational/experience requirements outlined in the NYS licensure laws.

Able to lift, push, and pull light to moderate loads.

Current Driver’s License, required car insurance and a car available for work.

KNOWLEDGE, SKILLS AND ABILITIES:
===============================
Extensive knowledge of the principles and practices of community health nursing and its relationship to other human services.

Superior clinical knowledge based on application, with in-depth understanding of case management and community resources/needs.

Analytical and technical skills needed to provide creative solutions to complex service-related problems requiring a synthesis of data from staff, patient and community population.

Interpersonal skills necessary to influence and interact effectively with others in order to implement agency goals and resolve complex service-related issues.

Ability to plan, organize, direct, administer, delegate and supervise.

Knowledge of regulations affecting licensed and certified home health agencies.
