Job Title: Program Assistant - Case Manager
Job Summary: Provides for clients’ well-being and independence by providing personal care and developing and facilitating therapeutic group activities; coordinating client care.
Reports To: Program Manager
FLSA Status: Non-exempt

1. Addresses client personal care needs by assisting clients in restroom, with ambulation, meals and hygiene as needed;
2. Determine clients’ situation and needs by establishing rapport with clients and caregivers; establishing service goals and plans; coordinating interdisciplinary assessments with team members;
3. Maintains continuity of care by communicating changes in client condition to team members; informing caregivers and physicians; documenting changes and service plan events; monitoring progress toward service goals and adjusting services as required;
4. Maintains a safe and healthy environment by adhering to infection control protocols; addressing potential safety and health hazards; following emergency policy and procedures; and fulfilling education and certification requirements;
5. Promotes client independence, education and recreation by developing and facilitating activities according to clients’ abilities and interests; encouraging interaction and participation;
6. Develops a safe, therapeutic group environment by establishing and keeping group rules; monitoring and encouraging appropriate participation; intervening as necessary;
7. Establishes a compassionate and respectful environment by practicing good communication skills; keeping client information confidential; conducting self in professional manner;
8. Maintains required documentation to meet WAC guidelines and/or Full Life Care policy;

1. Maintains personal care supplies inventory by checking stock level; anticipating needs; informing appropriate team member when supplies need to be ordered;
2. Obtains assistance for clients by coordinating care while at Full Life Care arranging for appointments; making referrals to other services;
3. Advises clients and caregivers by suggesting and exploring options; providing feedback and guidance;
4. Cross-train to work at day centers and boarding homes;
5. Complete projects and tasks as assigned by supervisor;

Educational Requirements Needed to Perform the Duties of the Job:
1. Bachelor’s degree in human services or related field and one or more years experience working with adults in a health care or social service setting, or high school diploma and two or more years experience in related field;
2. Must complete the HIV/AIDS information course;

Physical Requirements Needed to Perform the Duties of the Job:
1. Must be able to lift 25 lbs. and support clients’ mobility
