JOB TITLE: Registered Nurse/Case Manager

CLASSIFICATION: FT Exempt (Sign On Bonus Available)

REPORTS TO: Clinical Manager or Program Manger

Under the direction of the Clinical or Program Manager and as part of the health care team, delivers hands-on nursing care to patients. Assesses initi-ates, evaluates and reports on patient health status. Supervises Hospice aides as appropriate.

Under the direction of the Clinical or Program Manager, provides compre-hensive health care services to a caseload of patients and their families through direct care and supervision.

Requires a graduate of an accredited school of nursing.
Requires current RN license in the State of Ohio.
Two years of recent clinical experience, preferably in homecare, hospice, oncology, gerontology, or surgical nursing required.
Requires references indicating demonstrated skills and expertise.
Requires effective communication skills.
Requires the ability to learn computer applications needed to produce and retrieve a complex va-riety of information. Typing proficiency and computer literacy preferred.

Clinical/Patient Care
Performs comprehensive assessments of patients/families as required.
Develops and revises the patient’s plan of care in an accurate and timely manner per hospice reg-ulations and standards.
Informs the patient upon admission to HOCO of the mechanism for receiving, reviewing and re-solving patient complaints.
Uses assessment skills to determine patients’ physical and mental status and status of environ-ment and social support.
Maintains patient’s immediate environment to be safe and clean.
Identifies patients’/families health and social issues and potential problem areas.
Initiates referrals to other services/community programs as indicated.
Identifies present and needed resources (financial, human and material) of patients.
Implements measures to meet identified needs.
Develops measurable patient-centered goals in collaboration with the patient and family.
Implements services in accordance with the established care plan, standards of nursing practice and in compliance with third party regulators, hospice requirements, HOCO procedures, proto-cols and all federal, state and local laws/regulations.
Prepares, administers, and documents medications and treatments as prescribed in the Plan of Care.
Reports pertinent clinical findings to physician/ Clinical Manager/Program Manager as appropri-ate prior to leaving patient’s home.
Teaches self care techniques to patient/family.
Evaluates the patient/family response to the Plan of Care against established goals.
Provides supervision to ancillary health care workers as assigned.
Documents care/treatments provided and patient response in an accurate and timely manner and per HOCO policy
Completes medical records according to professional standards, third party regulatory require-ments and HOCO policy.
Completes other HOCO records and reports in an accurate and timely manner.
Adheres to HOCO infection control policies and procedures.
Implements the admission process for patients/families who are newly admitted to the Hospice Program.

Performs scheduled visits in accordance with HOCO productivity guidelines.
Ensures compliance with the Preferred Drug List and Pharmacy guidelines.
Participates and supports the QAPI program.
Attends and participates in HOCO meetings, Activities, programs, mandatory inservices, and continuing education classes.
Attends and participates in inter-disciplinary team meetings.
Remains flexible to changes in HOCO policy and procedure and in daily scheduling.
Maintains regular work attendance.
Contributes to effective consumer relations by assisting patients, families, co-workers, and other health care members to resolve expressed concerns through the utilization of a positive and help-ful attitude.
Keeps in strict confidence all communication and information regarding patients/families.
Is responsible for maintenance of own education needs as determined by the Ohio Board of Nursing.

Maintains regular work attendance.
Demonstrates professionalism by awareness of posture, dress, and language.
Complies with all applicable local, state, and federal laws and regulations.
Maintains confidentiality of all patient and agency information as outlined in Hospice of Central Ohio policy.
Demonstrates behavior which reflects the mission and values of Hospice of Central Ohio.
Treats all persons with respect regardless of race, color, national origin, age, disability, or sex.
Performs other duties as assigned by Clinical or Program Manager (immediate supervisor).
