Home Health by Hale Makua provides personalized treatment for medical conditions provided in the comfort of the client's own home. Those qualifying for services are functionally or medically homebound, in need of intermittent skilled nursing or therapy services, and have a Hawaii physician overseeing their care. Services include skilled nursing care, assessment and interventions, disease management, infusion therapy, terminal care, case management, skilled, physical and occupational therapy, speech pathology, and more.
The program has had numerous accomplishments in terms of quality and outcomes, and has received national recognition for excellent client outcomes and management. Home Health by Hale Makua is ranked in the top 8% in the nation for keeping clients well enough so that a return to the hospital is not needed. Most recently the Home Health Staff was presented with the Striving for Excellence Award from Mountain-Pacific Quality Health, the Medicare Quality Improvement Organization (QIO) for the State of Hawaii. The award was received for Home Health by Hale Makua's efforts in consistently maintaining a low acute care re-hospitalization rate for our home care clients. The agency had a very low rate when compared to other state agencies and the national rate. The QIO and all state home health agencies have made this a focus for quality improvement for the last several years.
Home Health by Hale Makua admits about 720 patients a year and has 85-100 patients on service on any given day. The Program also manages 24 foster family clients and assists the Maui PACE program with its home care needs. – less–ZoomInfo
