There was never enough time to chart (or document) effectively. Granted 20% of the patient's took 80% of the time. Regardless, it would have been easy to stay 2-3 hours after work and document. But we were always pressured to leave and an to accumulative overtime. It wasn't a matter of working smarter not harder. No, overall. the nurses were overworked.
