General Job Description:
The Targeted Case Manager is responsible for coordinating the delivery of services as well as determining eligibility. The position requires you to advocate for the acquisition of services and resources necessary to implement the service plan by representing or defending the client's service's through direct intervention, monitoring progress and evaluate the appropriateness and effectiveness of services.
This Certified Case Manager position requires a Bachelor's degree from an accredited university or college with a major in Social Work, Psychology or a Human Services related field and a minimum of two years experience working as a case manager.
Essential Job Functions:
Access for Medicaid services
Determine a client's functional eligibility by completing the assessment process
Develop the clients' plan of care within a cost effective manner and meet their basic health, safety, and welfare needs
Conduct home visits and telephone communication with clients' in order to ensure health, safety and welfare need or other services needed
Advocate for clients by arranging for services with individuals, businesses, and agencies for the best available service within limited resources
Ensure that clients have full access to a variety of services and service providers to meet their specific needs
Participate in the external and internal quality assurance process
Complete billing related documentation for the Medicaid billing department ensuring billing documentation is accurate and complete
Collaborate with the billing clerk department as needed to ensure accurate and timely reimbursement for client services provided
Participate in clinical review of client for services and enhanced placement when appropriate
Attend staffing on behalf of the client to ensure the needs of the client are being addressed fully
Attend and participate in trainings as required
Perform presentations to community agencies as requested
Participate in the quality improvement process
