The HCP of Texas RN Case Manager is responsible for the admission, planning, intervention, and ongoing evaluation of patients in home care. As an interdisciplinary team member, our RN’s provide excellent care and service that is safe, timely, effective, efficient, and patient centered. The RN Case Manager must have expert level clinical assessment skills and be able to make independent decisions for the well being of the patient. The RN must have knowledge of the OASIS and be able to complete a full assessment on a patient upon admission. Visits may include using such skills as vitals, medication review and education, IV administration, wound care, PT/INR, Foley insertion, patient and caregiver education on disease management and addressing safety concerns.
Home Care Providers of Texas believes in the RN/LVN team model. With this said, the RN Case Manager will be partnering with the LVN to schedule patients appropriately, thus giving the RN time for admissions. The RN will also coordinate care with therapy and Home Health Aides when necessary and ordered.
The RN Case Manager will utilize the electronic health record in the home to best capture and document patient information.
Registered Nurse with Associate Degree or Diploma in nursing.
Must be licensed by the State of Texas as a Registered Nurse; and in good standing.
Must have a minimum of one (1) year of experience in the Home Health Setting with Geriatric experience.
Must have the ability and willingness to rotate through the on-call rotation.
Must possess the ability to make independent nursing judgments with minimal supervision.
Must demonstrate sound and proven skills in performing clinical procedures, expert level in verbal, written, and electronic health records, and superior time management and organizational skills.
Must successfully pass a background check.
Must be able to show proof of the following items on the first day of employment:
- RN license in the state of Texas, in good standing
- Valid Texas Driver’s License
- Automobile Liability Insurance
- A reliable vehicle (driving is required with this position)
- CPR Certification
- 2 approved I9 documents indicating eligibility to work in the United States
