About Horizon Home Care & Hospice, Inc.
Horizon is a private, not-for-profit organization sponsored by Froedtert Memorial Lutheran Hospital, Columbia St. Mary's and Community Memorial Hospital. We are licensed to provide home care and hospice services, are Medicare and Medicaid certified and accredited by The Joint Commission since June of 1999. The Joint Commission conducted an on site survey – more... in June 2008. At that time, our agency was re-accredited. Our next accreditation survey is planned for 2011.

We serve patients in southeastern Wisconsin, averaging a census of 1,000 patients served by a staff of more than 550 employees.
Horizon's Home Care goal is to promote the ability of patients to care for themselves to sustain life and health or recover from disease or injury.
