What is your nonprofit’s history?
Hospice of the Foothills (HOF) is meeting the needs of more than 500 patients and families annually in western Nevada County and the surrounding communities. HOF provides hospice, palliative, and respite care regardless of ability to pay, as well as unreimbursed supportive services including bereavement care and community outreach.
Hospice of the Foothills (HOF) began serving patients with life-limiting illnesses and their families in Nevada and Placer Counties in 1979 with an all volunteer staff. To address a growing patient census, by the early 1980’s, paid healthcare professionals joined the volunteer staff including RNs, medical social workers, an executive director, and administrative staff. In 1994, HOF became a Medicare and Medi-Cal certified Hospice Agency. Throughout this early period, Hospice care was provided primarily in patient homes.
In July 2010, HOF opened its on-site, state-licensed, Medicare certified in-patient care facility; our Compassionate Care Home (CCH). The opening of the CCH resulted in the expansion of programs, services, and personnel.
Who is your primary audience?
Hospice of the Foothills serves the end-of-life care needs of the terminally ill and their families in their homes or the place they call home, including skilled nursing and residential care facilities.

Success Story: How your organization has benefitted the community:
100% of families we serve would recommend Hospice of the Foothills to others in need of hospice care. Hospice of the Foothills has provided end-of-life care for 33 years serving over 500 patients and families annually. We are the only hospice in our community providing Medicare Certified In-patient care directly. Therefore, the families we serve are more broadly supported in all care settings that include skilled nursing facilities, residential care facilities and most often in the patient’s or family’s own home. Our professional care team of hospice nurses, counselors and aides also includes the patient’s personal physician, our Medical Director and team physician who are board certified in hospice and palliative medicine.
In addition to our hospice programs and services, we support the community in the following ways: We give access to advanced care planning resources, provide a Transitions program at no cost for those needing advanced care but for whom hospice care is not a choice, offer community grief support groups, specialize in care to Veterans, and provide end-of-life education through our Speakers Bureau program.
