PURPOSE OF POSITION:
To work collaboratively with physicians and nurse practitioners within the team setting to provide appropriate education and support to clients with diabetes in accordance with contractual obligations as stipulated by Intercede Health.
EDUCATION, TRAINING AND EXPERIENCE (Includes knowledge, skills and abilities):
Required: Current valid registered nurse or Registered Dietitian licensed in the state of Texas
Current BLS / CPR certification.
Current CDE certification
Minimum of 2 years of applicable clinical experience
Proficient computer skills
Preferred: Baccalaureate Degree
Supervisor or Charge Nurse Experience
Disease Management experience.
CUSTOMER INTERACTIONS REQUIRED:
Interacts routinely with interdisciplinary team, employees, peers, management staff, senior management staff, vendors, clients and other healthcare team members.
Routine contact with patients, patient caregivers, physicians and other health care providers.
Telephonically communicates with and participates in on-site visits with clients, managed care / health plan representatives, service providers and vendors.
