About Livingston HealthCare

Based in Livingston, we have served all areas of Park County for over twenty years. Hospice care can begin immediately, even on the weekends. We are able to provide a wide range of services-from physical and occupational therapy to family counseling. Because we are part of Livingston HealthCare, there is a shared patient record with the clinic and the – more... hospital and a strengthened continuity of care for the patient. Our office is able to provide durable medical equipment and supplies to support the patient and the family. Livingston HealthCare also has services for home oxygen that are usually covered by the hospice benefit. There are many physicians, nurses, and other professionals who can give references for Livingston HealthCare's hospice services. Our staff can review your hospice benefit and answer any questions you may have. If you have private insurance, our business office can find out about your coverage and review this information with you. As a non-profit, we may be able to offer financial assistance for families who qualify. Our staff will work with the patient, family, and primary physician to develop a comprehensive plan of care. This plan will be updated and revised as the patient's needs change. The hospice team will be made up of the patient's family, caregivers, physician, and other professionals, including an RN Case Manager, LPN, Home Health Aide, Volunteer Coordinator, Volunteers, Medical Social Worker, Bereavement Coordinator, and Non-Denominational Chaplain. We are happy to provide you with information about the training and experience of those staff who are working with your family. The patient's physician is an important part of hospice care and will be involved in any way necessary. Respite care is provided at Livingston HealthCare and Livingston Health and Rehabilitation, both located in Livingston. The patient's physician can participate in care at both of these locations. Bereavement counseling and support is a part of our hospice service and continues for 12 months after your loved one passes away.
