Job Summary: To collect, maintain, and complete patient health information. Prioritizes, locates, pulls, assembles, electronically tracks, and sees to the appropriate transport of information requested by other departments.
This position reports to the Director of the Health Information Management Department.
Education: High School diploma, Registered Health Information Administrator, and/or Register Health Information Technician (RHIT).
Experience: Preferably one to two years experience in the Health Information Management Department of a hospital performing duties of Health Information Technician.
Physical Requirements: Duties include lifting and carrying stacks of medical records that may weigh twenty-five (25) pounds. Must be able to perform task, which require hand/eye coordination. Must be able to hear normal sounds with some background noises. Must be able to sit for prolonged periods of times up to 3 hours.
Skills: Must possess: excellent verbal and written communication skills, demonstrated skills in computer operations, excellent organizational skills, analytical abilities, reasoning skills to effectively sole complex problems, excellent human relation skills.
Responsible for discharging patients from the electronic medical record Carelogic.
Responsible for delivering requested information to the appropriate staff members.
Analyzes patient data for any delinquencies in the electronic medical record Carelogic.
Analyzes requests for medical information, evaluates legality of releases, abstracts pertinent portions of medical records, copies, mails and/or releases in accordance with departmental policies to safeguard patient confidentiality.
Notifies all staff members regarding deficiencies on what needs to be completed.
Responsible for maintaining the completion of deficiencies by the staff by the approved deficiency date.
Accurately file all paper chart(s) in appropriate areas in a timely manner.
Accurately files complete and/or incomplete chart(s) loose papers in a timely manner.
Responsible for reviewing and accurately processing requests for Release of Information.
Answer incoming calls for the facility in a professional manner.
Responsible for the processing and tracking of dictated Discharge Summaries, H&P, and/or Psychiatric Evaluations, as well as any other dictated reports (if applicable).
Responsible for the upload of the dictated reports received from the outside transcription service QED into the electronic medical record Carelogic.
Follows departmental procedures for archiving inactive paper records; pulls charts, prepares charts for laserfiche.
Scan charts into laserfiche in accordance with departmental policies.
Assist other Health Information Technicians with special duties.
Assist Director of Health Information Management Director with assigned special duties.
Performs miscellaneous job-related duties as assigned.
