We are recognized as the leader in Westchester in providing innovative care that involves collaborating with each individual to achieve his or her goals. The (Nurse) Director Care Management Program is responsible for overseeing and growing an integrated care management unit serving clients with complex behavioral and medical conditions. This position is responsible for staff development, care plan oversight, quality outcomes and multi-system operations. The successful candidate will have strong leadership skills, sound knowledge of chronic illness management and care management experience.
Ability to master an electronic record system, meet deadlines and productivity targets and willingness to grow and change are musts.
The candidate must be a licensed RN. A Nurse Practitioner is preferred. Candidates must consent to Fingerprinting and a Criminal History Information Records search.
