I have an assigned case load that can fluctuate on a daily basis due to their are certain diagnosis in the military that are automatic case management for active duty. We can do spouses and dependents but our primary objective is to take care of an injured active duty, That can be from overseas, accident in vehicle, or complex illness that requires case management to navigate through the referrals, MD offices, clinical notes, and required military paperwork. Primary objective is to get the person back to work or assist in them getting boarded out of the military. Need to contact patients can be from monthly to daily depending on the acuteness of their situation. Constantly talking with hospitals, rehab, and offices in getting care set up for the patient and making sure all the referrals and paperwork is being done to take care of them. Primary care managers are on site and we frequently talk to them throughout the day to keep them updated, review notes, or tell us their orders for care for the patient.
Have my own office and have privacy to do my job. Salary is good.
Frequent interruptions can occur causing difficulty with accomplishing tasks.
