North American Health Care, Inc. in Dana Point, California, is seeking experienced full-time Clinicians (LVNs, RNs, NPs and Physician Assistants) to serve as Case Managers at our client skilled nursing facilities (SNFs) throughout California, Washington, Arizona, and Utah. The positions are based at our client facilities and require computer proficiency to efficiently review electronic medical records. Appropriate applicants are dynamic, detail-oriented licensed clinicians who are experienced with medical care and documentation in acute or post-acute settings. The primary focus of our Case Managers is to verify accuracy and completeness of medical records based on clinical reviews, best practices, and patient interaction. Each Case Manager may serve multiple facilities within a geographic region. Daily travel by car is typically required. Therefore, you must have reliable transportation, auto insurance, and a clean driving record.

Our client facilities are sophisticated, higher-acuity skilled nursing facilities with an emphasis on serving post-surgical, post-operative patients who have had extensive care in the acute care hospital and are now in need of sophisticated skilled nursing care. These facilities have a special emphasis on serving orthopedic patients and other post-surgical patients who are recovering from a planned surgery, or unplanned surgery, and expect to return home (or to a lower level of care such as assisted living or board and care facility) as soon as possible. Many patients’ average length of stay is approximately two to four weeks. Some skilled residents only stay a few days, while others stay for up to three months. Additionally, our client facilities have long-term care populations and those patients have extensive needs that we participate in serving as well.

As a Case Manager you would focus on many aspects of the peer review process for our service focused company, including reviewing clinical documentation to verify the accuracy and completeness of it through clinical reviews and patient interaction. Each client facility is responsible for providing all aspects of care, documentation, compliance and customer satisfaction. The Case Manager will verify and report consistency with all policies and procedures related to clinical documentation and patient care. You will carefully review the clinical documentation completed by the facility staff and community physicians, interact with patients and families, and then provide recommendations to the Service Center and the client facility in accordance with company guidelines and policies. You will be required to perform at a high level and report on a set minimum number of patients on any given day. As such, you must be focused and efficient to be considered for the position.

Qualifications:

  • Licensed clinicians including LVN, RN, NP or PA;
  • Knowledge of medical documentation and legal issues;
  • Computer proficiency and typing/data entry skills;
  • General office skills including filing, organizing, etc.;
  • Clear oral and written communication skills;
  • Ability to work within a team; and
  • Interest in maintaining professional development and continuing education on health information and legal issues.

Specific responsibilities include:

  • Conducting peer review of documentation by our client serviced facilities, including review of medical records and verification often involving patient interaction all in accordance with set guidelines and set minimum number of patients each day, week and month.
  • Reporting findings to your supervisor on a daily, weekly, monthly and quarterly basis.

Hours, Compensation and Location

Hours: M-F, full-time, salaried
Compensation: $15 - $45 per hour based on experience and license category
Location: Variable/Local Travel Required – California, Washington, Utah, and Arizona
