We are currently seeking qualified Nurse Practitioner medical professionals to provide Clinical Care Visits in the following cities in Akron, Cleveland, Toledo, Cincinnati & Lima OH areas. SUMMARY OF DUTIES & RESPONSIBILITIES
The Nurse Practitioner (NP) effectively identifies, evaluates, and addresses disease prevention and health promotion for Medicare Advantage members while conducting health assessments in the member's primary place of residence.
The Provider will work in an independent and interdependent relationship with members of the Outcomes Health Connections network of Providers. Collaboration between Outcomes Health Connections internal staff, including recruiting, project management, scheduling, collaborating physicians, and the Clinical Director will be required.
The Provider may also be required, to conduct in-home assessments which focus on the recognition and documentation of acute, chronic, and long-term healthcare issues. Diagnostic Screening tests may be performed based on client need.
The Provider will educate patients and/or families about preventive care, medical diagnosis and use of prescribed medical treatments and/or medications as needed.
Documents and tracks accurate and confidential health risk assessment using technology based application or paper documentation tool.
Comprehensive Physical Exam to include all body systems: ADL and Fall Risk Evaluation, Depression and Nutritional Health Screening, BMI evaluation and diagnostic laboratory screenings.
Must be available a minimum of 2 days per week consecutively between the hours of 8-5pm Monday-Sunday.
Providers will report to Outcomes Health Connections Clinical Director.
Under the general supervision of a collaborating physician will be based on individual state advance practice regulations.
REQUIRED EDUCATION & EXPERIENCE
Preferred 2 years of clinical practice as a board certified Nurse Practitioner.
Nationally board certified in Family Practice, Adult or Geriatric medicine required.
New Graduates accepted on an individual basis if practicing for one year in geriatric medicine.
Master’s degree required in specialty.
CERTIFICATION OR LICENSURE
National Board Certification required – ANP or ANCC.
Must be state licensed to practice within the health plan contract state.
BLS-current, proof of certification required.

Medicine - Knowledge of the information and techniques needed to diagnose and treat injuries, diseases, and deformities. This includes symptoms, treatment alternatives, drug properties and interactions, and preventive health care measures.

Reading Comprehension - Understanding written sentences and paragraphs in work related documents. English language required.
Oral Comprehension - The ability to listen to and understand information and ideas presented through spoken words and sentences. English Language required.

Customer and Personal Service - Knowledge of principles and processes for providing customer and personal services including needs assessment techniques, quality service standards, alternative delivery systems, and customer satisfaction evaluation techniques.
Critical Thinking - Using logic and analysis to identify the strengths and weaknesses of different approaches.
Bilingual in Spanish preferred but not required.
