About Parsons Child and Family Center
Parsons Child and Family Center serves as the lead agency for the Home and Community Based Services (HCBS) Waiver in Upstate New York, which includes Albany, Columbia, Fulton Montgomery, Greene, Hamilton, Rensselaer, Saratoga, Schenectady, Schoharie, Warren, and Washington counties. Youth ages 5 to 17 years with serious emotional disturbance and complex health and mental health needs are most often eligible for this program. These youths must also require institutional level of care, be at imminent risk of admission to an institutional level of care, or have a continued need for hospitalization. Youth must also have service needs beyond what one agency or system can provide, be capable of being cared for in the home or community if services are provided, and have a viable and consistent living environment with parents or guardians who are able and willing to participate in the HCBS Waiver to support the youth in the home and community.
HCBS Waiver program is a cost-effective approach designed to provide services that enable youths to remain at home or in the community, decreasing the use of institutional placements. Staff use a strengths-based individualized care approach to working with families. The Individualized Care Coordinator (ICC) works with the family to identify natural supports and resources that may serve as additional supports or advocates for the youth or for the family. HCBS Waiver services available to families include individualized care coordination, respite care, skill-building services, intensive in-home services, crisis response services, and family support services.
The program's professional staff include master level human service professionals, all experienced in working with youths who have serious emotional disturbance. These staff are trained in individualized care coordination and intensive in-home service delivery. The program manages several subcontracts for Waiver services, which allows families a choice of service providers as they shape their youth's service plan.
