GPPC practice site managers are generally either an LPN or RN , chosen for proven ability to direct staff and provide feedback to clinical providers as needed. Site managers are responsible for the general workflow of individual sites as noted below in job duties. Site managers are an integral part in care teams and providing input to achieve better patient outcomes and achieve quality measures. Managers have excellent knowledge of principles of documentation, HIPAA, and medical records management protocol.


  • Initiating or directing a staff member to provide weekly provider schedules
  • Adequate staffing and vacation coverages
  • Provide training for new employees
  • Assist all employees with duties as needed
  • Ability to assess employee strengths and weakness and assign staff positions appropriately
  • Provide corrective measures resulting from unsatisfactory job performance
  • Report to senior practice administrator as needed
  • Team leader or care management team
  • Reinforce team concept
  • Hold monthly care team meetings and document meeting notes
  • Familiarization with General Physician, P.C. billing staff and human resources managers to promote good communication
  • Hold clinical in-service programs in accordance with clinical co-ordinator’s training schedule
  • Direct staff in emergency situations
  • Runs reports to monitor for access and communication policies
  • Address safety issues
  • Perform or designate employees to carry out chart audits
  • Monitor results of audits and initiate procedures for continued quality improvement
  • Expertise with Medent system with ability to follow established protocol
  • Expertise with DM/HM screens, including customization and overriding of measures of care
  • Undergo training of PQRS, Meaningful Use, PCMH, NCQA, DRP and incentive programs
  • Understanding of HEDIS measures
  • Providing suggestions and input to administrative co-ordinators
  • Address office problems and provide appropriate feedback
  • Hold quarterly provider meetings, reviewing pertinent topics
  • Adherence to confidentiality
  • Provide appropriate educational information and referrals to community services
  • Understanding concepts of pre-planning, goals, and barriers
  • Ability to identify vulnerable populations and patients impaired with vision and hearing functions
  • Knowledge of running quality reports within Medent system
  • Ability to designate assigned duties to secondary members of care team
  • Ability to utilize triage system within Medent to notify primary care physician or mid-level of patient related issues, e.g. non-compliance, abnormal test reports, etc.
  • Review current pricing of immunizations and any medications ordered and report changes to billing staff
  • Understanding of how to use the To-Do system to notify secondary care team members to carry out follow-up tasks
  • Ability to work with patients in a caring and empathetic manner
  • Understands general principles of chart documentation
  • Follows HIPAA protocol when speaking with patients


  • Valid NYS nursing license, LPN or RN
  • Well versed in primary care patient care
  • Documented certificates of continuing education programs and Webinars
  • Required attendance at in-service programs
