Assists residents with activities of daily living including bathing, grooming, dressing, toileting, changing of bed linens, positioning in and out of bed, chair, etc. Ambulate and transfer residents utilizing appropriate assistive devices and body mechanics. Prepares residents for meals and snacks, assists residents in eating where needed and records food intake. Measure and record residents' heights and weights. Take temperatures, pulse, blood pressure and respirations as directed by the supervising nurse. Assists residents with recreation programs. Reads and follows care plans; performs assigned restorative and rehabilitative procedures; reports changes in residents condition to nurse in charge; documents care provision on resident record/flow sheets as required; reports accidents and incidents; reports suspected and alleged incidents of abuse per policy; and provides nursing functions as directed by supervisor. Understands and follows safety and sanitation rules, and promptly reports any violations to supervisor. Keeps assigned resident bedside area clean at all times and personal belongings in orderly arrangement. Attends all orientation and in services as required by the state. Understands, complies with and promotes all rules regarding residents' rights and maintains confidentiality of information and the medical record. Promotes positive relationships with residents, visitors and government regulators to include presenting a professional appearance and attitude. Other duties as assigned.
