Southridge Healthcare is currently expanding operations and under new management. As a result, we have a variety of positions open at this 106 long term care facility located in Sioux Falls, SD. Our new Post-Acute Rehab facility will be opening in the Fall of 2013. We currently have Charge Nurses (RN's), House Supervisors (RN's) and MDS Case Manager positions available.
The House Supervisor is responsible for coordinating and maintaining quality resident/patient care through effective management of the activities of the total resident care during assigned shift. In the absence of the Administrator, DON and/or designee, the House Supervisor has the authority and responsibility for administrative decisions and appropriate notification of the administrator on-call. This is a 12-hour shift with weekend and holiday involvement.
The MDS (Minimum Data Set) Case Manager is a registered nurse (RN) that conducts federally mandated assessments for the residents at Southridge Healthcare. The MDS Case Manager is responsible to formulate and implement individual care plans for residents. The MDS Coordinator will gather information from residents and their families during initial and periodic interviews. This is an 8-hour shift.
The Charge Nurse performs the primary functions of an RN including assessing, planning, implementing and evaluating the care of a designated group of residents in a designated time frame. Demonstrates knowledge of gerontological residents physical, social, emotional and psychological needs based upon the residents health care needs. This can be an 8-hour or 12-hour shift. Full and Part time opportunities are available.
All positions require licensure as a Registered Nurse in the State of South Dakota. The level of experience required depends upon the position applying for.
