The four communities are: Storm Lake, Humboldt, Pocahontas and Sac City. In some locations, acute care inpatient services are provided by institutions that are not part of Trinity Health Systems.

In response, Trinity Regional Medical Center worked with its parent organization, Trinity Health Systems (THS) to develop Physician Practice Management Corporation (PPMC), a wholly-owned, for-profit subsidiary of THS for the purpose of providing management services. PPMC continued to grow and expand over the course of seven years. In December 1996, PPMC acquired nine specialty divisions of the Fort Dodge Medical Center and began pursuing enhancements of the regional network to ensure access to and deliver of the best quality care in each medical setting. As a growing physician organization, PPMC changed its name to Trimark Physicians Group in March of 1998.
