RN Case Managers needed to plan, implement and assess the plan of care for each patient in the caseload and utilize principles of community health nursing to provide skilled nursing care to patients.
Perform skilled assessments of patients including physical, psychosocial and environmental factors.
Formulate a written Plan of Care and ensure appropriate documentation is completed to admit patients to service.
Perform and administer skilled nursing care within the scope of nursing practice.
Provide instruction and supervision to patients and caregivers regarding the patients' care and health status.
Demonstrate ongoing patient assessment by making necessary changes in the plan of care.
Monitor the patient's response to the care provided and consult with the physician regarding the patient's response, changes in the condition and changes needed in the plan of care.
Assume responsibility for a caseload of patients in a defined geographic area and/or a specified area of expertise. Ensure the development, implementation and coordination of each patient's Plan of Care.
Accept responsibility for problem solving for patients in the caseload, including follow up on calls from patients or calls regarding patients.
Oversee the scheduling of nursing visits ensuring continuity and coordination of patient care.
Ensure utilization of nursing specialites and other disciplines in meeting patient care goals. Maintain awareness of care provided.
Current CA RN license.
One year of full time employment as an RN.
Experience in a medical/surgical acute-care setting preferred
Previous home care experience preferred.
