Position Summary

Working in a matrix style environment, the Advance Practice Provider works directly with the faculty and other medical professionals (departmental, institutional and external) to accomplish goals, objectives and activities broad in scope as listed below.
Leadership: Demonstrate flexibility and accountability for continuity of care in a dynamic environment. Develop and raise performance levels to facilitate patient outcomes. Implement patient care delivery systems and standards of care. The position is integral to the success of the Department’s clinical practice and research program at NYPH and WCMC as well as our national position with Peer Institutions.

Position Activities

*1. Clinical Practice and Patient Care: Direct patient care within the scope of licensure; manage the care of patients in our clinical practice areas. Establish a triage role to provide information and appropriately route patient questions and referrals. Review and document patient medical information, such as intake surveys and medical history. Obtains and documents subjective history (interval or comprehensive) as appropriate to chief complaint or reason for visit. Conducts relevant physical exam, documenting pertinent positive and negative findings. Triage and interpret laboratory results and radiology studies. Write prescription(s) according to New York State and federal law. Maintain constant communication with patients and their families regarding pre-visit requirements and diagnostic testing and pre-and post- surgical care and follow-up for the long-term. Effectively identifies, evaluates, and addresses disease prevention and health promotion issues of the population in the practice while administering quality patient care.

*2. Administrative: Develop, monitor and refine critical pathways and clinical protocols for improvement of patient outcomes: demonstrate ability to prioritize patient care activities. Maintain appropriate patient medical records and other institutional documents. Complete medical forms and medical correspondence related to patient activity and restrictions.

*3. Education: Patient & Family Provide educational information regarding surgery and treatment plans as proposed by the attending physician. Provide patient and family education and counseling on health status, treatment alternatives, complications and possible outcomes. Educates patients and/or families about preventive care, medical issues, and use of prescribed medical treatments and/or medications

*4. Staff In-Service: Communicate with attending, house staff and referring physicians and other healthcare professionals regarding department and program activities and conducts in-services and training programs as needed.

*5. Vendor Relations- Interact with and maintain relationships with various vendors regarding equipment and pharmaceutical needs. Work with Peri-operative Services to procure/acquire necessary equipment to perform surgical procedures.

*6. Research and Studies: Coordinates clinical research studies; maintains comprehensive databases for patient outcomes. Assists in developing appropriate quality assurance design for potential use in research protocols. Ensures optimal quality of medical care for patients participating in clinical trials. Perform H&P on potential and enrolled subjects. Monitor patients participating in clinical trials for toxicity, adverse events and medical complications. Provide counseling and education to participants.

*8. Preceptor ship: Serves as a clinical preceptor for nursing, nurse practitioner or physician assistant students relevant to area of specialty.

Performs other related duties as assigned.

Minimum Requirements

*Current New York State license to practice as a Nurse Practitioner; Master's preparation with certification in appropriate discipline required; Family Nurse Practitioner preferred
2 years related experience required clinical setting; academic institutional setting preferred; Demonstrated competence and excellence in clinical practice ;
Protection of Human Subjects Training required ;Current DEA license, NPI, NP BCLS and/or ACLS required.
Previous experience with EMR, EPIC/ECLIPSYS highly desirable
