Welcome to Winchester House

Winchester Place has a rich history of providing healthcare for the residents of Lake County for over 150 years. As of August 1, 2015, The Lake County Board selected Transitional Care Management ("TCM") to provide ongoing, high quality health care and rehabilitation services for the residents of Lake County. TCM will serve as Winchester House’s management company with the three-year goal of building a brand new center in a yet-to-be-determined Lake County location. At that time, the new facility will then be licensed and operated as Transitional Care of Lake County.

Now is an exciting time to be a part of the Winchester House team as we plan for the future. This will include serving the current residents with care, compassion and outstanding service while also growing our post-surgical, more medically complex, short-term rehabilitation services.

Comprehensive Services

Winchester House offers twenty-four hour skilled nursing care, intermediate care, short-term rehabilitation and memory care. Join us as we plan for the future of Winchester House where we will capitalize on our rich history of serving the community with high quality, comprehensive short-and long-term care services and prepare for the future by growing our post-surgical, more medically complex, rehabilitative, post-acute care.

Focused Rehabilitation Services

The Winchester House rehabilitation program helps individuals return to their highest level of function following an injury, illness or surgery. Our progressive therapy team provides focused rehabilitation services seven-days-a-week and achieves outstanding functional gains with our patients.

This is an in-house therapy program, not a contract rehab agency. As a new therapist joining our team, you will be able to be a part of building programs, shaping protocols and have a caseload filled with diverse diagnoses, including the medically complex. We have a fully equipped gym and an experienced multidisciplinary team for you to collaborate with.

Life Enrichment

Winchester House offers an enriching activities program and provides an engaging quality of life for our residents. Activities are planned to help residents continue favorite hobbies, find new and satisfying relationships, celebrate holidays and explore or develop new interests. Residents also travel to community outings and special events and many community organizations donate their time and talent to provide musical concerts, special programs and socialization for the residents.
