You have to take the USMLE in order to meet their standards. But passing the step 2 there in the Phil does not guarantee you a visa. I have friends who did and were denied. Visa sponsorship is quite hard since most employers would not or would prefer a resident here, if not, a U.S. citizen.

I think the wisest thing to do is to get here first. Then, you can plan on just anything. Good luck!
