Health Insurance at Prudential Financial

My spouse is interviewing with Prudential Financial, and I'm curious about the health insurance. The interviewer is saying they are VERY good benefits with VERY good coverage. But what does that mean?

Can anyone tell me a little more about who the health insurance is through, and what the coverage is like? I'm specifically interested to know if:

1) Weight loss surgery is covered?
2) Follow up to weight loss, plastic surgery?
3) Pre-existing conditions?
4) What is the plan an HMO or PPO or ... ?
