Criminal Background Questionaire - T-Mobile

Does anyone recall exacly what the questions where that pertained to the criminal parts of the background questionaire?

With Verizon it was 7 years older on felonies, you didn't( DID NOT ) have to reveal.

Misdemeanors, I believe it was 3 or 5 years or lder for convictions that you (DID NOT) have to reveal.

Also, how does T-Mobile feel about DWI's and if they where nearly 5 years. This Sept 24th will be 5 years for my DWI.

I also have a Driving while license invalid thats 4 years old. misdemeanor.

If someone can tell me what those questions stated exactly. That will help me better prepare my information.
