401(k) Retirement Plan
• Company match on your contributions up to 6% of earnings
(Certain restrictions may apply, company match ranges $0.50 - $1
for every $1 you contribute based on plan eligibility.)
• Your contributions are fully vested when made
• Company contributions are fully vested no later than after 3 years of
service. (Vesting period is based on plan eligibility.)
• Company-provided annual contribution of 3%-5% of pay based
on age and years of service.
• Annual contribution is 100% vested after 3 years of vesting service
• You direct how your plan accounts are invested
• Benefit is portable
Life Insurance
• Basic term life insurance,
o No cost to employees who are non-tobacco users.
(Tobacco users are required to pay a premium).
o Equal to 2X your annual salary;

Medical Benefits (PPO Plan)
First day coverage – Choice of plans
• Plan A – 80/20% co-insurance*, $250/$500 deductible, $1,250/$2,500
maximum out-of-pocket
• Plan B – 80/20% co-insurance*, $450/$900 deductible, $2,250/$4,500
maximum out-of-pocket
• Plan C – 80/20% co-insurance*, $800/$1,600 deductible, $4,000/$8,000

Comprehensive medical/Rx plan

Network Pharmacy
Co-pays range from $15 - $70 when using a RESTAT Network Pharmacy
There is a $100 cost share* for high cost drugs in all networks.
• Non-network pharmacy co-pays are 50% of total prescription cost.
• Mail Order for 90-day supply of prescription drugs is also available.
*For each prescription and/or refill of prescription drugs costing more than $1,000,
your cost share will be $100. If the cost of a 90-day mail-order supply of a prescription
drug exceeds $3,000, your cost share will be $300.

as a company employee you only get paid once a month, and personally for me, the position I had in engineering didn't pay enough if it wasn't for the bonus, which was 10-14% at that time, but its a once a year check that is heavily taxed, if your young and have the time to work up its okay, I ended up quitting and going back to work as a contractor and made 20k more with the same benefits and less corporate BS (such as personal profile, enternal resume, smart goals, safety goals), all that BS that keeps you from doing your real job.
