I just got this email yesterday on how to transfer this is what was sent to me hope it helps Thank you for your inquiry to the Department of Public Health, C.N.A./H.H.A./C.H.T. Certification Unit. In order to transfer your certificate over here to California , you will need visit our website to download the reciprocity packet. The reciprocity packet consist of the Initial Application (HS 283B), Sample Request for Live Scan Service form (BCII 8016 Sample), the Request for Live Scan Service form (BCII 8016) and the Verification form (CDPH 931). You will need to submit the completed application, a copy of your out-of-state certificate, the completed live scan form and proof of work that you have provided nursing services one day for pay within the last two years. If you have received your initial certificate within the last two years then you do not have to worry about the proof of work. The CDPH 931 is to be completed by applicant and the endorsing state agency. Once we receive your application, we will put you into our system, we verify you are active and in good status in the other state and you may begin to work. You will receive your certificate once we have all of this along with your fingerprint clearance. I was passed my cna tests and became certified in New York and moved to Arizona and transferred it to there. but now im moving back to new york next week. It says im still certified in NY from the website but can you really be certified in two states? |

Hi, you can call your state public health dept,call information to get the phone number, if you live in another state other than calif,I believe you need to take classes again to be certified by the state here,otherwise if you have a califonia certificate to CHHA,as I do, was told, you can use that in any other state..but always, check out your own information,things can change, and people can be misinformed..like myself...LOL email address for california is (cna@cdph.ca.gov...good luck 3/21/12 |
