I have been in the dental industry for 20+ years. At this point in my career, I would like to transition into Consulting as opposed to Practice Manager. I'm not sure how to make the change. Any advice?

Positioning yourself as turn around expert could be one option, it is my experience that many Drs wait for major problems to occur before seeking out consulting help.
The marketing of consulting services is the critical component, as you are obviously qualified for the dental part.
Maybe offering specific services, on a part time basis, might be an answer.
