What could cause the stored information corrupted in NAND FLASHes?

In one of my designs, data in the NAND flash myseriously corrupted. It doesn't happen often, in the range of two one thousandths and it seems that all the corruption happened after a write to the chip. I am baffled. Any input is appreciated.
