For people that have applied for thier CA CLS license?

I am thinking about applying for the CA CLS license but am wondering what forms I will send when I apply.

Will LFS require only my transcript?

And how do they verify that you have the full year of hospital training? Will I be required to contact my school to get the forms that my hospital instructors signed off on? Because that is a lot of forms.

Also, I have 9 credits of Physics from a college I went to 7 years ago. Will those physics credits be accepted?
