Advice: How does one go from being a contractor to being hired as a permanent employee or as a "direct" hire? I have been working as a contractor for 10 years, due to family commitments, and also graduate school. Any advice?

I been both - contractor and permanent (direct hire) most of the time the company initiated the request to change status to permanent this is beneficial to you since you know they want you therefore you have the upper hand. So you hear what they have to offer you and then you request what you would like.
If you are a contractor and you are the one who want the permanent position; just apply to HR (if position exist) and mention your desire to convert to permanent and wait for the hiring manager to contact you -- the negotiations are in their side but not necessarily but either.
