Contact the department of Health and Sr. services office of the state of Illinois. In Missouri you can check you're certification number on the state web site.

Contact the Department of Health and Sr. services in Texas. They should be able to fax the info over to the office that issues the CNA certifications in Newark Delaware.

Check the state web site of the state you are moving to. Each state has their own regulations, on transfering CNA certifications.

you have to take the kansas statre boards in kansas and I think you have to pay $50 but I think you have to get ahold of the kansas state board of nursing in topeka to schedule an appt date.I currently have a Ks cna that I am tring to get transfered to Tx

also if u need to make a name change in the search field type in form 5508

I have a 9yr.old son with C.P, Epilepsy, VNS, trah and feeding tube. We are entitled to nursing 24/7. Right now we have a nurse for 4 hours 3 times a week. Why can't we get more help? I am a single parent and I am on the verge of a burn out. We need a nurse who will work with Medicaid but it seems impossible to find any help.

IT'S PROBABLY REALLY HARD BECAUSE THE AGENCIES AND MEDICAID DON'T WANT TO PAY THAT WELL. THE BEST THING TO DO IS HAVE THEM PAY YOU TO CARE FOR YOUR SON AND THIS WAY YOU KNOW HE IS GETTING THE BEST.

Your License number should be same as your Social Security number. Illinois registry web is hcwbc.idphnet.com/BgChecks.Public/Search.aspx Put first 3 letters of your Last and First name to find out if you are registered. Good luck

I am going through the same thing right now but I have not yet recievied my drug felony and no one will hire me what do we do! I an almost going crazy about the whole situation! So to let you know you are not alone~

Generally, they won't pay you to care for your own child however. Medicaid will only allow 15 hrs of care per week if you are contracting directly with the Home Care Agency. However, your state Social Services department generally has a program that will allow double that time or better. In NC, the program is called CAP (Community Alternative Programs)
