If you already are Licensed, Certified, Registered or State Approved in your own State and want to move to another state, you will need to request reciprocity. This means that you are asking the new state to recognize the approval you received from your home state. Contact the State registry in your home state and request a "Application for Enrollment By Reciprocity". Ask them if you should send the completed form to them or to the state to which you are moving. The contact information for the state registry can be found at www.4cnas.com www.4cnas.com

i moved to fl. from london,ky in 93- so i'm sure things have changed--florida use to recognize other states cna certifications--all i had to do was challenged the state test and only paid 25, the nursing home i was working at paid me back--like i said things change. here is an --this is who you're going to contact to change your certification--state of florida-department of health-division of medical quality assurance-i'm sorry i don't the phone number where are u moving to in florida? |
