For the CDHO Clinical Competency Evaluation, how the patient's calculus should be?

They say, Subgingival interproximal calculus must be present on most posterior teeth.
I am wondering if they count a rough surface as a calculus which is not shown on x-ray film? or they count only the chunk calculus?

What kind of patient was rejected? Please tell me your experience.
I do not go to school here, I do not have an any instructor to ask.
Thank you.
