Look online for the certification company name. My card was unreadable. I contacted Esco Institute, gave my personal info, they found it, and with a small fee $15.00 paid my new card is in the mail.

You need to know the card issuer name or the name of the testing company. They will have your file and credentials. Contact them via phone or internet. Cost around $15 to get new one.

I've tried here in ga at supply house they have checked my social in the data base and they say no record i have to retake the test
